Services and Support Programs
We Provide
Service and
Support Administration
Service and Support
Administrators (SSA) provide a continuous support system to eligible individuals. These services include Individual Plan development for
Medicaid Waivers, Supported Living and Workshop skill development
tasks. Support is delivered on a 24/7 basis to meet the needs all
participants.
Early Intervention Services
Early Intervention (EI) services are provided to children ages birth through 2 who have, or are at-risk of having a developmental delay. After permission is received, a family/child assessment is completed to determine eligibility. Services are provided as described by the child/family Individualized Family Services Plan (IFSP) that is developed by Help Me Grow.
Services are individualized to meet the unique needs of each family and child and are available in the family /caregiver home, or center. Home-based services are available year round. EI services are provided under the guidelines set by the Ohio Department of Developmental Disabilities; 5123:2-1-04.
Vinton
Industries
A Workshop setting is available
for eligible individuals to receive Habilitation training to promote independent skill development. Job skills are enhanced in the
Workshop setting through contract work opportunities and through
Community Employment at various sites within Vinton County. With
the support of family members and volunteers, numerous social events occur throughout the year.
Transportation
Transportation is made available to eligible individuals to and
from Vinton Industries and the community work sites.
Additionally, a VCATS system is available to participants and other
members of the community who need transportation to services outside
of Vinton County.
Family Support
Services
Through money provided by the
Ohio Department of Developmental Disabilities, income eligible individuals and members of their
families can access funds for
adaptive home improvements or to
purchase needed items that promote an independent
lifestyle.
Supported
Living
Limited funds are available
to individuals who need additional residential support and who are not eligible for a Medicaid Waiver. Money can be used to pay a
Service Provider to assist with
training in the development of
personal and independent living skills. The amount of money provided is based on the individual and the level of support needed
to promote independence
and ensure all health and safety
needs.
Medicaid
Waivers
Through an agreement with the
Ohio Department of Jobs and Family Services and the Ohio Department of DD, Medicaid funds are available to qualifying
individuals. Each participant must have a level of care that
requires a support system for them to successfully live in the
community. Individuals can then select a Service Provider and with
the support of team members, determine their personal needs and
necessary level of support.
